understood that package deal prices are
not subject to any further discounts and
are for procedures performed by hospital
listed medical practitioners only.
Please note that wherever more than one
procedure is carried out under the same
anesthetic that main procedure will be
charged in full and 50% discount will be
applied to the second procedure and each
subsequent procedure.

Admission:
A
patient
being
admitted
from the
outpatient
clinic
to the
hospital
shall be
classified
as
either
routine
or
emergency.

explains
the need
for
admission
and the
admission
plan to
the
patient
and/or
family,
including
expected
length
of stay,
any
surgery
to be
done,
benefits
&
complications
of
surgery,
drawbacks
if
patient
refused
to
undergo

cost
attached,
and
available
package
deal
systems.

The
consultant/specialist
will
fill the
Admission
form
order in
the
patient’s
medical
record
and
instruct
the
nurse to
escort
the
patient
and/or
family
to the
admission
office,
where
he/she
will
fill the
admission
form and
proceed
with the
admission
process. |

Emergency
|
|
|
»
The
consultant/specialist
explains
the need
for
admission
and the
admission
plan to
the
patient
and/or
family.
»
If the
patient
is for
surgery,
the
consent
form
should
be
completed
and
signed
by the
patient.
If the
patient
is a
child,
the
father
or
mother
must
sign the
consent
form. At
least
one
witness,
other
than the
physician,
must
sign the
consent
form.
The
consultant/specialist/nursing
staff
places
the
completed
consent
form in
the
patient’s
medical
record
file.
»
The Head
Nurse on
the
admitting
ward is
informed
of the
impending
admission
& all
relevant
data,
concerning
patient's
name,
age,
sex, and
admitting
diagnosis
and a
bed
number
obtained.
»
The
Admission
Officer
will
provide
a bed
number
for the
patient
after
communicating
with the
head
nurse of
the
admitting
ward.
»
The
patient
and/or
family
are
escorted
to the
Admission
Office
(if the
patient's
condition
permits)
along
with the
medical
record
file.
»
The
patient
is taken
to the
ward, by
wheelchair
or
stretcher
if
necessary. |
